Advance Care Planing(ACP) is for any competent 18+ adult.
Its about what you want as a written legal document and able to be shared in a medical emergency.
She asked all present at the meeting who had an ACP.
Ony 2 said they had.
Without an ACP if a patient is unconscious their Enduring Power of Attorney (generally a close family person or friend) makes decisions on their behalf.
This won't necessarily be based on what you may have actually wanted i.e resuscitation when you may not have wanted it if you've had a severe stroke as an example.
The ACP can be backed up by another document or orally recorded statement: an Advanced Directive which is also legally binding clarifying your wishes.
The ACP can be scanned into Health Records including GP records.
St Johns have fridge magnet cards indicating that you have an ACP and a wallet card with the same info.
Organ donations are not covered by the Plan.
Mary gave everyone an ACP Information Pack to provide guidance for its preparation.
In thanking Mary Geoff Harding commented that her address was very thought provoking and timely for the audience.
